Hematology-Oncology (CHO) is a select group of physicians and other medical
professionals specializing in the diagnosis and treatment of cancer and blood
diseases. Our professionals have committed years to training, research and
teaching to  remain at the
leading edge of our profession and technology.
We are

CHO Launches New Online Patient Registration System
CHO is proud to launch its Patient Registration System. The system is designed to allow patients to complete their registration form prior to their appointment. This system will save you time and will eliminate the need to complete the form at the office. The registration system is completely secure, so your information is safe. All patients are required to fill out a new registration form annually.
Here's how the system works:
The registration form is one page and can be filled out on-line. Please have your insurance card and pharmacy phone and address readily available, as that is required information on the form. The completed form will be submitted directly to our office. Once the registration is complete, you will need to download and print three forms for you to sign and bring in with you to our office.
